Health Loop pty ltd recognises your right to privacy, and your right to control your personal information. We take the security of your confidential data very seriously, and we are committed to protecting all personal information that we hold under the Australian Privacy Principles (APPs), as set out in The Privacy Act 1988. We thank you for trusting us to take care of you, and your privacy.
Our privacy management plan promises to:

PERSONAL INFORMATION
Privacy Act definition: Information or an opinion about an identified individual, or an individual who is reasonably identifiable:
a. whether the information or opinion is true or not; and
b. whether the information or opinion is recorded in a material form or not.
Collection & Disclosure:
We are providing rehabilitation services and therefore may collect sensitive personal and health information via telepractice or in-person delivery. This ensures our services can be undertaken and delivered within the best practice guidelines, abiding by the code of professional conduct and ethical practice.
During your dealings with Health Loop services, we may collect, store, and securely disclose (only to relevant service providers) personal information such as;
Name, address, contact, and employment details
Health funding information such as Medicare, DVA, and private health details
Medical history, referral letters, reporting, and clinical notes.
Any relevant clinical information in regard to your presenting injury, condition, disability.
Relevant information from service providers involved in your rehabilitation such as GP, Specialist, Solicitor, Employer, Insurer, and other Allied Health Professionals.
Please note, whilst we take all reasonable steps to ensure professional, legal, and ethical obligations to protect confidential information are met, total privacy cannot be guaranteed when using third-party provider technology during telepractice.
If you provide personal information in regard to another individual you will need to ensure you are authorised to. You will also need to informed the individual of our company, and that we may use and disclose their personal information in accordance with our privacy policy (i.e. when providing emergency contact details).
Storage & Security
Dealing with sensitive personal information and health records shapes every decision we make at Health Loop. It is imperative to us that your personal information is stored securely and protected with high levels of data privacy measures. Your data is encrypted and stored on our electronic database with high grade security compliance. We are a paperless organisation, and therefore we do not store any personal information outside of this secure electronic system to avoid loss, interference, misuse, or unauthorised access. Any hard-copy records we may obtain are destroyed.
Health Loop is legally required to securely withhold certain personal health records, and after the obliged time period has ended this information will be de-identified, deleted, or destroyed where practicable.
All third-party services and contractors such as IT providers are required to comply to the privacy laws, and respect the confidentiality of all personal information held by Health Loop.
Access Rights
You have the right to request and access your personal information Health Loop stores. Please contact us directly via phone or email and we will deal with all requests to access personal information as diligently as possible.
Health Loop reserves the right to refuse access to personal information as permitted under the Privacy Act 1988. There are ten grounds listed where Health Loop can refuse to allow access, which include:
We reasonably believe that giving access would pose a serious threat to the life, health or safety of any individual, or to public health or public safety
giving access would have an unreasonable impact on the privacy of other individuals
The request for access is frivolous or vexatious
The information relates to existing or anticipated legal proceedings between Health Loop and the individual, and would not be accessible by the process of discovery in those proceedings
Giving access would reveal the intentions of Health Loop in relation to negotiations with the individual in such a way as to prejudice those negotiations
Giving access would be unlawful
Denying access is required or authorised by or under an Australian law or a court/tribunal order
Health Loop has reason to suspect that unlawful activity, or misconduct of a serious nature, that relates to the organisation’s functions or activities has been, is being or may be engaged in and giving access would be likely to prejudice the taking of appropriate action in relation to the matter
Giving access would be likely to prejudice one or more enforcement related activities conducted by, or on behalf of, an enforcement body
Giving access would reveal evaluative information generated within Health Loop in connection with a commercially sensitive decision‑making process

ONLINE PRIVACY
Health Loop services will occasionally collect personal information from you via electronic forms, or when you access our website. This information may be securely collected and uploaded to our electronic record system to be utilised within your rehabilitation services. Any personal details submitted will only be used for the purpose for which they were provided.
Your phone or email address will be recorded if you contact us. Your contact details will only be used for the purpose intended, and it will not be added to a mailing list or used for any other purpose without your consent.
By using our website you accept the use and installation of cookies. We may use this information for additional functionality, to analyse usage patterns, and other statistical purposes. Cookies does not usually directly identify you, as it is generally de-identifies and does not retain personal information. However, it may include the IP address of your computer, as well as give you a more personalised internet experience. You can turn cookies off in your browser setting.
COVID-19 POLICY
Our COVID safe policy is subject to all regulation, standards, and guidelines as directed by the WA government and Public Health authorities. For more information please contact the Department of Health WA. Health Loop employee’s are fully vaccinated, and we encourage all clients to stay up to date with their vaccination status.
CODE OF CONDUCT
Health Loop and our employees remain proud of our strong ethical standards, inclusive culture, and professional conduct with clients, their families, and caregivers. We are determined to continue growing and learning whilst maintaining these high moral standards and values across all aspects of our business. If these standards are not met, and a breach of conduct is evident, Health Loop will take the necessary disciplinary actions.
